What is body contouring?
Body Contouring; Nowadays, due to aging, giving birth or gaining and losing weight, there may be an increase in body fat, the formation of excess skin and sagging due to these excesses. All of the surgeries performed to correct such deformations are called body contouring surgery.
The increase in body deformations due to inactivity, improper and malnutrition caused by modern society increases the demand for body contouring surgeries.
Today, the desire to change in body perception and approach perfection, which is increasing for many reasons, also makes body contouring surgeries more popular. It should not be forgotten that in order to preserve the changes in the body after body contouring surgeries and to be long-lasting, it is also necessary to make changes in nutrition and lifestyle.
How is body contouring performed?
Body contouring surgeries have a sequence within themselves. This order may vary from person to person and according to patient needs. It is more common worldwide to start body contouring surgeries from the torso area. The torso area is the main area for surgeries.
Torso surgeries basically include liposuction and stretching procedures to be performed on the abdomen, breast surgeries, stretching and liposuction procedures to be performed on the back and waist area. Body contouring surgeries performed on the torso area rapidly change the patient’s appearance.
In this way, both the quality of life and comfort of patients increase and patients become more motivated for other surgeries. After the body contouring surgeries performed on the torso in the first sessions, arm and leg surgeries can be performed in the other surgery sessions. This order can be changed according to the patient’s priorities and needs.
For example, the arm area can be included in the first session surgery in patients who have complaints about their arms, who cannot use their arms sufficiently due to finding clothes or the weights on their arms. Again, if the patient cannot move sufficiently due to the weight and sagging skin on the legs, including the legs in the first surgery session will increase the patient’s comfort of life.
In surgeries such as fat injection to the butt area, butt lift or butt prosthesis, it should be kept in mind that the patient should lie face down after the surgeries and planning should be done accordingly.
Fat injection procedures to the buttocks are usually performed together with liposuction surgeries. Considering that the fat to be used in fat injection can be removed with liposuction, it is imperative that the two surgeries are performed together.
In patients with sagging buttocks, buttock surgery is performed together with tummy tuck and liposuction surgery. The abdominoplasty procedure is extended towards the waist area in the back and a 360-degree tummy tuck is performed and the buttocks are lifted upwards.
During this procedure, the buttocks can be given a rounder appearance with the fat taken from the patient. Thus, at the end of body contouring surgery, the patient can have a butt that is both upward and volumized.
For patients who do not have enough fat in their body, butt prosthesis can be used in body contouring surgeries. Regardless of which procedure method is preferred, patients should not lie on their backs in body contouring operations including buttock surgeries.
Therefore, it is important to plan the surgery well. In patients who cannot lie face down or in patients who should not lie face down due to the combination of surgeries, I use specially made BBL beds. These beds have a gap in the butt area. In this way, even if the patient lies on his/her back, there is no pressure on the patient’s butt area.
Which surgeries does body contouring include?
Body contouring surgeries include all surgeries to be performed on the torso, arms and legs. To list from top to bottom, breast surgeries, arm liposuction and arm lift procedures, torso liposuction and tummy tuck surgeries, leg liposuction and thigh lift surgeries and surgical procedures to be performed on the buttocks are included in body contouring surgeries. Here, which surgery will or can be performed on which area varies from person to person. The main goal in combinations of surgeries is to achieve harmony between body parts.
What is local body contouring?
Local body contouring procedures are surgeries performed under local or general anesthesia for problems in only one of the areas mentioned above.
For example, the fat in the upper part of the inner leg can be removed by vaser liposuction method under local anesthesia. The same procedure can be performed for local fat areas in the knee. Again, tightening that area with Renuvion j plasma procedure against mild skin sagging in the leg area is another example of local body contouring surgeries.
Local body contouring interventions can be performed against local fat areas in the arm and abdomen areas. After the fat in these areas is removed with liposuction, skin tightness can be increased with j plasma.
What are body contouring procedures?
Fat filling (fat injection):
Fats taken from the body by liposuction can be purified through certain processes and injected into other areas as fillers to give volume. There are 3 parameters to be considered in fat injection. These are where to take the fat from, how to turn it into pure fat and how to inject it into the appropriate area.
The most permanent fats in the body after injection are the fats taken from the area below the belly button and the inner thighs. It is generally preferred to use fat from these areas in body contouring surgeries.
The fats taken with the appropriate liposuction method contain blood, tissue particles and fluids given during liposuction. All these substances are separated from the fat tissue with the help of special filters. The prepared pure fat is then passed through filters of different thicknesses to make it injectable.
In this way, the fat becomes both liquid and has different thicknesses. It is important that the fat has as little contact with air as possible during the purification phase. There are specially planned closed systems for this purpose. In addition, the oils prepared before injection can be enriched with stem cells or (Platelet Rich Plasma) obtained from adipose tissue to increase their permanence.
The fat prepared in shaping surgeries can be injected into the buttocks, breasts, the inner part of the area below the knee to thicken the legs, and into the chest, shoulder and arm areas for a more athletic appearance in male patients.
Approximately 40% of the fat used in fat injection is absorbed by the body. The remaining part remains in the injected area for a lifetime. In order to reduce the absorbed portion and increase the permanence of the fat injection, it is important to purify it under appropriate conditions without contact with air. In the same way, a higher rate of permanence is obtained in fat injected by mixing with stem cells or fat.
Tummy tuck
One of the most important parts of body shaping surgeries is tummy tuck (abdominoplasty) surgery. Abdominoplasty is an operation that tightens the abdominal area and makes it look flatter and athletic.
Over time, sagging and stretch marks may appear on the abdominal skin of women due to births or rapid weight gain and loss. These sagging and stretch marks indicate that the quality of the abdominal skin has deteriorated. Especially in women who have given birth more than once, we hear a lot of complaints such as I cannot get rid of my belly no matter how much I lose weight.
The most important reason for this is that the abdominal muscles (rectus abdominis muscle), which move further apart during each pregnancy, cannot return to their former state (rectus diastasis). For this reason, tummy tuck surgeries should focus on the abdominal skin that has lost its elasticity and the abdominal muscles that are separated from each other.
In tummy tuck (abdominoplasty) surgeries, the skin that has lost its elasticity under the belly button is removed and a more taut and smooth abdominal skin is obtained. In addition, the abdominal muscles that are separated from each other are brought closer to each other with special suturing techniques, resulting in the formation of tight abdominal muscles.
The most common question our patients ask is what happens to the belly button. The belly button is actually a pit and is connected to the abdominal muscles from its base, so the location of the belly button does not change during surgery, the belly button separated from the skin around it remains where it is, while the belly button, which remains under the abdominal skin after the excess skin is removed, is taken out with an incision made just above it and adapted to the skin around it again.
Another issue that our patients are curious about in tummy tuck surgeries is the scar that will occur after surgery. When planning abdominoplasty surgery, care should be taken to ensure that the scar that will form after surgery remains in the patient’s underwear.
Abdominoplasty surgery, which takes about 4 hours, can be combined with vaser liposuction on the side of the abdomen, lower and upper back to provide a slimmer and more beautiful waist appearance. In abdominoplasty surgeries performed together with breast prosthesis and breast reduction surgeries, an ideal shaping of the upper half of the female body can be achieved.
After abdominoplasty surgery performed under general anesthesia, the patient must stay in the hospital for at least 1 night. Our patients, who are asked to walk slightly bent at the waist for the first few days, can return to their normal lives within a maximum of 10 days.
Arm lift
Arm lift surgery is an aesthetic operation performed to remove thickening and sagging in the upper arm. Increased adipose tissue and excess skin may occur in the upper arm due to weight gain and loss and aging. While only excess skin is seen in some patients, the increase in adipose tissue in the arm may be accompanied by heaviness of the arm and excess skin. Arm lift surgery is an important part of shaping operations.
If the patient has excess skin and increased fat tissue in the arm, arm lift (brachioplasty) surgery can be combined with vaser liposuction. With vaser liposuction, the fat tissue accumulated in the arm is removed and the excess skin is removed using an elliptical incision starting from the armpit and extending to the inner elbow area.
In this way, both the arm is thinned and the excess skin is removed. By removing the excess fat in the arm, the weight of the arm decreases and accordingly, the tension on the suture lines decreases, which provides a better wound healing. The incision on the arm is relatively less visible because it is located in the inner and posterior region of the arm.
It is possible to get rid of the fat in this area and tighten the arm with vaser liposuction, especially in young patients who do not have excess skin and have good skin quality.
Vaser ultrasonic effect provides tightening of excess skin with the heat effect it gives while liquidizing the fats. In addition, J plasma technology, which converts helium gas into plasma energy and tightens the skin, can be used to tighten the skin more. J plasma is very effective on the arm due to the appropriate skin thickness.
Arm lift (brachioplasty) is an operation that takes about 2 hours and is performed under general anesthesia. We keep our patients in the hospital for 1 night so that they can recover from the effects of anesthesia. After arm lift surgery, we recommend our patients to use an arm corset for up to 6 weeks.
After the surgery, our patients can return to their normal lives within 1 week. Patients can start cardio exercises 10 days after surgery. Exercise movements that require arm use can be performed 6 weeks after surgery.
Arm lift surgery can be performed alone or in combination with operations such as tummy tuck, breast lift, thigh lift in our patients who have sagging in other parts of their body due to high amounts of weight loss as a result of recent bariatric surgery. The general condition of the patient, blood values and the size of the surgery to be performed should be taken into consideration when performing these surgical combinations.
Thigh lift (thigh lift)
Thigh lift surgery is a preferred surgery to be performed after shaping the torso and abdomen in body shaping surgeries. Depending on weight gain and aging, fat and excess skin can be seen in the leg area.
These excess skin usually occurs in the thighs. In body contouring surgeries, only liposuction can be performed on the thigh area or liposuction and thigh lift can be performed together.
In patients with only increased fat in the inner thigh and knee area, liposuction into the thigh may be sufficient. If the skin quality is good in these patients, excess skin does not occur. In addition, for these patients, J plasma can be applied to these areas to increase the skin firmness in the inner thigh area.
J plasma tightens the connective tissue under the skin with the plasma energy it delivers under the skin. With this effect, the formation of excess skin is prevented and a more natural tight surgical result is obtained.
In patients with increased adiposity and excess skin in the thigh area, liposuction and thigh lift are performed together. In this surgery, the leg is first thinned by liposuction. Afterwards, excess skin is removed with thigh lift.
In thigh lift surgery, the scar remains on the inside of the leg. The length of the scar varies from patient to patient according to the amount of excess skin. Since the surgery is performed under general anesthesia, patients need to stay in the hospital for at least 1 night. When performed alone, the thigh lift operation takes about 3 hours. When combined with other surgeries, the duration of the operation varies.
Drains placed after thigh lift surgery are usually removed 4-5 days after surgery. Our patients need to use a corset for about 6 weeks after this surgery. Patients who return to their normal lives 10 days after the surgery should wait 6 weeks to start sports.
Mini Vaser
Gaining weight or having an overweight body is one of the biggest problems of modern man. Men and women gain weight in different parts of the body due to genetic and hormonal reasons. While women usually gain weight in the lower abdomen, hips and legs, men gain weight in the upper body and abdomen. Liposuction is an integral part of contouring surgeries.
With the introduction of fat-specific devices in recent years, liposuction has transformed from a standard fat removal procedure into a shaping operation. The most important of these devices are Vaser (ultrasonic liposuction) and PAL (power assisted liposuction) and Renuvion- j plasma methods.
In the Vaser liposuction method, only fat tissues are damaged by using ultrasound waves. While the adipose tissue becomes liquid, the vessels, nerves and connective tissues around the adipose tissue are not damaged, so less bruising and faster healing is observed after surgery.
The fats melted with the Vaser method are removed from the body with vibrating cannulas in the PAL method. With J plasma, the skin under which the fat is removed is tightened and excess skin is prevented. In this way, irregularities and fluctuations in the skin after liposuction, which were observed in the past, are prevented.
In body contouring methods, also known as Hi-Def or 4D, it is possible for patients who are close to their normal weight but have regional adiposity to achieve a more athletic body structure.
In men, it is possible to achieve a six-pack appearance by shaping the fat in the abdominal area with the Hi-Def (4D) body contouring method. In order to provide a balanced body appearance in the patient whose abdominal area is made more muscular and thin, a muscular body appearance can be achieved with fat injections into the chest, arm and shoulder areas.
With the Hi-Def (4D) body contouring procedure in women, a suitable body shape can be achieved by giving an athletic appearance to the abdominal area and thinning the waist and back area. It is possible to obtain a thinner waist and wider and rounder hips by injecting fat from other parts of the body into the buttocks.
With mini vaser surgery, very small regional fat deposits in the body can be easily removed under local anesthesia. The best example of this is double chin liposuction surgery. In this surgery, the double chin area can be easily removed under local anesthesia and patients can return to their normal lives at the end of the day.
In my own patients, I perform Hi-Def (4D) body contouring surgeries using Vaser, PAL and J plasma methods together. In this way, I want to minimize the irregularities that may occur after surgery and achieve a faster recovery time and better body shaping.
In these surgeries performed under general anesthesia, the duration of surgery varies between 2-4 hours depending on the number of areas where the procedure will be applied. We keep our patients in the hospital for at least 1 night so that they can fully recover from the effects of general anesthesia and feel better.
After the surgery, we ask our patients to use the corsets we put on after the surgery for 6 weeks so that the skin under which the fat tissue is removed can fully fit in its place.
